The Clinical Case Manager at O’Connor Professional Group (OPG) works with clients in their homes and communities, helping to coordinate and execute an interdisciplinary plan of care. Clinical Case Manager also act as liaisons between clients, family members and healthcare personnel. Clinical Case Managers will meet with their clients multiple times per week in the community or home settings to monitor their progress and coordinate services. By providing support, and advocacy, Clinical Case Managers empower clients to achieve their own personal, recovery, and professional goals. The Clinical Case Manager will also be responsible for marketing on behalf of OPG to various referral sources and at relevant events.
Coordinating and helping to complete a comprehensive assessment addressing the client’s psychological, educational, vocational, social and recreational needs and review of past evaluations and reports
Clinical: defining what clinical services will be accessed, and how frequently and efficient communication process and monitoring client’s engagement in services
Educational: assistance in selecting a school program, support of a client’s school plan, from class selection to connections to school-based resources such as tutoring and counseling
Vocational plan: assessment of vocational interests, long-term planning towards career goals and short-term, tactical support for finding and keeping a job
Recreational plan: discovery of client’s interests in cultural, sports-related and artistic pursuits. Connections to appropriate resources.
Social: working with youth around how to pursue healthy behaviors and assistance in developing a positive support network.
Independent living skills: creation and monitoring of a budget, assistance with time management and other basic living skills as needed.
Creating short and long-term personal goal plans (related to above); daily tracking of progress
Knowledge of processes and procedures for documenting client progress
Providing connections to endorsed community resources and residential/outpatient treatment options
Completing necessary documentation
Drug testing, alcohol monitoring medication management
Providing logistical support (transportation, helping clients find suitable living environments and financial management)
Finding connections to peer counselors and recovery companions as appropriate
Participating in an on-call rotation
Assisting with special projects/administrative support as deemed appropriate
Emergency Support as needed
Other duties as assigned
Master’s degree required in social work or related field
