Community Behavioral Health (CBH), part of Philadelphia’s award-winning Department of Behavioral Health & Intellectual disAbility Services (DBHIDS), has an exciting opportunity for a Clinical Care Manager. The Clinical Care Manager is responsible for ensuring that children, adolescents and adults receive timely and medically necessary behavioral health treatment. The Clinical Care Manager conducts initial and concurrent reviews for behavioral health care treatment that includes inpatient psychiatric treatment, inpatient substance abuse treatment, behavioral health rehabilitation services, acute partial and residential treatment. This position also assesses for risk factors, identifies individuals requiring care management services, and guides the discharge planning process with treatment providers and coordinate continuity of care. Primary Responsibilities Include • Conduct initial and concurrent telephonic reviews for admissions into behavioral health care services using established medical necessity criteria • Concisely document clinical rational • Ascertain treatment progress since admission • Identify proposed treatment plans, risk factors and special needs • Determine client specialized service needs • Assist treatment providers in developing appropriate discharge plans • Participate in interagency meetings to assist in coordinating access to the most appropriate and least restrictive treatment • Coordinate with respective Office of Mental Health, Office of Addictions Services, Office of Mental Retardation Services and the Department of Human Services to ensure that supplemental benefits such as case management, recovery housing, vocational services, and entitlements are provided This is a full time position located at our offices in Philadelphia’s historic district. Requirements
Qualified applicants must possess Master’s Degree and/or Advance Degree in Social Work, Counseling, Psychology, Rehabilitation Counseling, or equivalent, and a minimum of three (3) years experience in assessment and/or counseling within an adult or child/adolescent behavioral health treatment setting, including but not limited to: Mental Health, Substance Abuse, Mental Retardation, Physical Disabilities. Candidates must be able to type a minimum of 20 WPM. Philadelphia residency required.
