::The Program Therapist provides Social Services activities and specific treatment goals to targeting patient population as delegated by the Director. Is able to formulate clearly defined goals and objectives as evidence by writing goals that reflect the patient's presenting problems as seen in assessments. Conducts patient care monitoring; Presents accurate information to all team members. Provides therapy notes that reflect an awareness of the patient's pathology and problems, notes are goal directed. Ability to demonstrate knowledge of developmental tasks of children, adolescents and adults. Completes social history on each assigned patient; ability to recognize and evaluate behavior, changing levels of depression, tendencies toward self-aggressive behavior, cognitive functioning deficits, learning deficits and patient's capacity for self care. Treatment plans, updates and final treatment plans are completed timely, per hospital guidelines. Performs other Social Services duties aimed toward therapeutic care and treatment of assigned patients.
