Outpatient Social Worker/Therapist

BASIC FUNCTION:
Provides individual, family and group psychotherapy for selected outpatient and serves as a liaison/consultant with referring clinicians for patients of all ages. Committed to continued professional development and the acquisition of further knowledge/training as needed. Participates in training and research activities conducted by the program and is active in various facets of program development.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Conduct individual, family and/or group psychotherapy with selected outpatients and partial hospital work, including planning, preparation, and facilitation of group psychotherapy and psych education/ life-skills groups.
Provide case management services for the families or individuals assigned to caseload, including arranging, coordinating, and monitoring the provision of a broad range of supportive services to families from other community agencies, and maintaining a complete and up-to-date case record for each family or individual in assigned caseload.
Formulate and implement individual and family treatment plans designed to strengthen intact families, reunite divided families, stabilize the placements of children in long-term foster families, and support individual growth.
Work as a team with a staff of psychotherapists, psychologists, and psychiatrists for regular consultation, collaboration, and referral development, including functioning as a member of the interdisciplinary team and responsibly exercising independent judgment in assigned duties.
Create referral sources through building relationships with primary care, psychiatrists, EAP counselors, school counselors, and other providers.
Attend case conferences, program meetings, and meetings with outside agencies involved with cases; liaise/consult with referring clinicians.
Ongoing monitoring of patient risk assessment/management.
Detailed knowledge of hospital systems, including intake/admission procedures, experience in working with third party payors, and when needed, provide necessary clinical information to insurance companies.
Knowledge of normal and abnormal human development and its impact upon physical, emotional and behavioral development.
Provide appropriate feedback to consulting Psychiatrists, including documenting patient participation/progress, maintaining contact with outpatient providers in medical record, and correction of medical record deficiencies as necessary.
SUPERVISORY RESPONSIBILITIES:
If licensed in DC as LICSW or other clinical professional, may supervise unlicensed staff.
