Peachford Hospital is seeking a seasoned Outpatient Therapist.
Located in the Dunwoody neighborhood of Atlanta, Peachford features individual units for children, adolescents, adults and seniors, serving residents of Georgia and beyond. We offer inpatient acute care, as well as partial hospitalization and intensive outpatient programs. (www.peachford.com)
The Outpatient Therapist will provide quality direct and indirect patient care to individuals with substance abuse or chemical dependency problems.
Key Responsibilities of the Outpatient Therapist include:
Assess patientsâ€™ state and extent of chemical use or psychiatric state.
Conduct psychosocial assessments to document history and identify preliminary issues for treatment focus.
Develop and implement a plan of treatment, in conjunction with members of the outpatient team.
Conduct didactic and other appropriate patient groups, utilizing 12-Step philosophy, and/or psychiatric coping mechanisms.
Communicate with referral sources and other appropriate parties regarding patientsâ€™ progress.
Develop and coordinate an individualized discharge plan for the patient by utilizing treatment team and chart information to determine the patientsâ€™ aftercare needs.Identify and assess family or community resources such as group homes or mental health practitioners.
Participate in discharge process by completing summaries making aftercare referrals, and by attending discharge conferences with patients, family members, and treatment team members.
