Job Summary: The Behavioral Health Care Coordinator functions under the Behavioral Health Unit and reports to the Director of ED and Behavioral Health. The Behavioral Health Care Coordinator is a highly responsible and specialized direct service clinical worker functioning as a screener serving a variety of populations who have mental and substance abuse issues. The Behavioral Health Care Coordinator works in the ED and the BHU and serves as a liaison between the two departments to coordinate admissions and care of behavioral health patients. Work involves screening and assessing patients for appropriate psychiatric disposition, obtaining authorizations for services and treatment, and making referrals to community agencies. Works as an extension of the Behavioral Health Unit staff to assist with patient needs on the BHU. Performs initial diagnosis in accordance with DSM-V criteria; performs screening for Baker Act Criteria or other established program criteria; gathers information for psycho-social assessments, mental status exams, and treatment plan; documents patients’ mental health history; presents information to admission committee/psychiatrist; makes referrals for discharge planning and transition of care. Coordinates Baker Act placement and discharges to Baker Act Receiving facilities for patients in the ED and the BHU. Participates in the Tele-Mental Health consult process. Maintains communications with necessary outside agencies and institutions to assure the patient continuum of care beyond the primary care physician and hospital. Utilizes effective communication techniques to promote patient, and family compliance with the anticipated plan of care. Demonstrates competency within the scope of practice, as appropriate to the ages of the patient services (neonate, child, adolescent, adult, and geriatric) and maintains knowledge of growth and development with an understanding of the range of treatment needed by these patients. Work is performed in accordance with established policies, procedures, and state and federal regulations. Work is reviewed through observation, presentations, discussions, clinical supervision and case records.
