Additional Key Elements/ Responsibilities:
Provide information and assistance to individuals seeking resources via phone and e-mail. This includes a) access, clarify and determine needs for each caller; b) based on needs identified, provide caller with community resources, agency services and disability information needed; c) problem solve and think creatively, as resources are limited; d) serve as liaison for TN Commission on Aging and Disability by providing referrals to potential program services recipients; e) write up assessments summary and plan for each caller in Call Sheet records.
Assist with updating agencies, resource lists and identifying new services in the community by a) assist with research on disability topics identified as needed and write reports to be distributed; b) assist with research and identification of new community services to be added to the database; c) assist with updating agencies that provide senior services by checking with the Area Agencies on Aging and Disabilities (AAAD) to ensure the database reflects current resources for older adults with disabilities, as well as resources related to COVID-19.
Community Engagement and Training by a) participate in selective community meetings to network and identify clients and/or services, especially Supporting families Lifecourse and Aging and Disability courses available through AIRS; b) provide training presentations at various local, regional and statewide meetings/conferences, especially TN Commission on Aging and Disability and AAAD staff; c) contribute relevant information to Pathfinder's social media network website.
Work collaboratively with staff in the immediate office and other VKC programs.
