Clinical Team Coord
Job Summary: Manages interdisciplinary medical teams to ensure continuity of care, case management and client education. Uses appropriate resources to understand specific disease processes, governmental regulations and guidelines of specialty associations in order to optimize patient outcomes. CREATES RELATIONSHIPS WITH PATIENTS, FAMILY MEMBERS, CAREGIVERS AND COLLEAGUES TO GAIN FULL INSIGHT AND KNOWLEDGE OF THE PATIENT AND THE FACTORS WHICH ARE AFFECTING THE PATIENT. * Considers patients first and demonstrates courteousness to patients, their caregivers, colleagues and others. * Utilizes effective interpersonal skills to initiate, cultivate and maintain friction-free relationships within the department and with external departments, physicians and agencies. *
