Your day.

As a Social Worker you need to know how to:

• Provide continuing care assessments, reassessments of patients and families to assist in development of their treatment and discharge plan; identify psychosocial / financial needs; access appropriate services and resources.


• Coordinate discharge / treatment plan with medical, interdisciplinary staff to effect timely discharge.


• Provide community resources for psychosocial / financial / educational needs of patient /family. Collaborate with agencies.


• Provide patient / family education, community education, i.e. advanced directives, domestic violence, child and elder abuse.


• Communicate / collaborate with medical, interdisciplinary team staff, patient / family, insurance companies, social agencies to provide quality discharge plan to appropriate, safe, level of care.

Your experience.

• Masterâ™s degree in Social Work or equivalent with experience in Discharge planning and counseling, required


• Minimum of two (2) years of related experience, required
