This position functions as an interdisciplinary team member, assisting physicians and other professional/non-professional personnel in a whole health partnership. Performs a variety of roles both in person and via telehealth, that include welcoming Veterans into the local system, orienting them to Whole Health concepts, offering a variety of points of entry into partnership with the local Whole Health System, coordinating and connecting Veterans to Whole Health resources and related programs.
