Overview Atlantic Health System is seeking a per diem Social Work for the Care Management Department at Morristown Medical Center to work 8am-4pm including one weekend shift per month. Responsibilities Essential Functions/Responsibilities:Responsibilities will include discharge planning for post-acute care needs.Insurance authorizations for sub-acute and rehab facilities.Conducting psychosocial assessments, family meeting and acting as an integral part of reducing LOS.Counsels, assesses, and assists individuals of all ages and families or groups who are affected by illnesses or injuries, including rape, physical abuse, cancer, AIDS, and Alzheimer's, that may impair their ability to function normally. Provides psychosocia
