The Binghamton VA Clinic is recruiting for a Care Coordinator Social Worker for the Behavioral Health Interdisciplinary Program (BHIP). The incumbent will provide case management, advocacy, and coordination of care within and outside the VA by collaborating with mental health and interdisciplinary treatment teams. The incumbent will conduct mental health triage and treatment matching assessments for Veterans, intake sessions for high-risk individuals, and provide Crises intervention when needed.
