This position functions as the Case Manager for the Health Care for Homeless Veterans (HCHV) Program, within the Social Work Service Line. The HCHV Liaison is responsible for monitoring and coordinating services provided by community based programs funded under VA's Homeless Providers Contract Housing Program.
