The incumbent is responsible for case management, coordination and provision of social work services for the Mental Health Outpatient Clinic. Responsibilities include a broad range of skills and services including completing psychosocial history and assessments, advance directives, treatment planning, case management, individual, couples and group counseling as well as the coordination of discharge plans for veterans to ensure seamless transition of care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
