Community Health Worker - Seaford
Nemours is seeking a Community Health Worker to support our Seaford, DE location. Position is 40 hours per week. The Community Health Worker (CHW) will be responsible for helping Nemours Children's Health patients and their families to navigate and access community services and other resources and adopt healthy behaviors.
