We are seeking a Social Work Navigator/BSW to support two unique Nemours programs! This role acts as an internal referral coordinator and liaison to Make-A-Wish to ensure that all patients eligible for a Wish are offered the opportunity. Functions as the hub between patient/ families, the Nemours Medical Team and the Make-A-Wish local organization. Attends team meetings, huddles and special events to provide education and awareness of the program resources available. Follows patients throughout the process and collects relevant data. Also acts as a navigator and coordinator of psychosocial resources for patients served by the Swank Autism Center including linkage to community-based resources, financial assistance, educational advocacy.
