Assesses needs for discharge planning and continuing care/resource support following discharge; independently makes recommendations to patients and families regarding post-acute level of care needs and options including:    Acute Rehabilitation Placement   Nursing Home or Skilled Nursing placement   Psychiatric or Substance Abuse placement   New Dialysis   Child/Adult/Domestic Abuse   Home Health/Hospice Referrals   Legal issues (adoptions, guardianship)   Assistance with Advance Directives   Community Resource needs   Financial Issues/Funding options   DME Referrals and Coordination   Social Determinants of Health       Ensures appropriate communication and updates are provided to the patient/family and members of the healthcare team and are documented as necessary to assure continuity of care.   Provide appropriate interventions which demonstrate knowledge of and sensitivity toward cultural diversity and the religious, developmental, health literacy, and educational backgrounds of the patient population.   Provides information and support to patients and families, helping them access needed resources within the medical center and community.   Ensures and maintains plan consensus from patient/family, physician, and payor.   Collaborates with the physician and other health care professionals to promote appropriate use of medical center resources. This role plays a key part in supporting early identification and intervention planning for children with developmental and behavioral health needs. 
 Clinical Child Psychologist,  experience and ability to treat children and adolescents who present with trauma and safety concerns, development delays, autism, and neurodevelopmental disorders. 
 QUALIFICATIONS 
 Education:  Ph.D. or Psy.D. in Psychology from an APA accredited graduate school required. Experience:  Two years clinical experience which may include the year of internship required. The second year must be post-graduate and include supervision by a licensed Psychologist or Clinical Psychologist. Licensure, certification, and/or registration:  Licensed by Virginia by the Board of Medicine as a clinical psychologist or by the Board of Psychology as a Psychologist (Clinical), or in West Virginia by the Board of Examiners of Psychologists required. Must have de-escalation training successfully completed within four months of hire and then maintained annually going forward when working in the inpatient environment. Other Minimum Qualifications:  Meets credentialing requirements. Ability to work cooperatively and professionally with physicians and other health care providers. Must have a commitment to continuing education.
