Case Manager RN - Peds Rehab - Part Time/Days - $7,500 Sign On Bonus SUMMARY: Experienced external candidates may be eligible for a sign on bonus up to $7,500. This position is accountable for assessing, planning, implementing, coordinating, monitoring, and evaluating options and services available to meet a patient's health needs. Coordinates services provided to a specified group of patients, ensuring that the care is provided in an appropriate setting, and that the severity of illness and the intensity of service is consistent with the patient's clinical condition. Collaborates with the health care team members to ensure that the care provided meets desired patient outcomes with appropriate utilization of resources within expected time-frames
